Such a thing can happen if your competitors don't use the key phrase in their titles. If your competitors are poorly optimized, please don't be surprised getting suggestions that differ from standard on-page optimization techniques (reduce the amount of text on the page, move the key phrase away from the beginning of a page element, reduce keyword density to 0 and the like). In some cases it's better to use the competitors' lack of optimization to your advantage while in others it's safer to keep to a lower optimization level (you'll be able to increase it gradually later as you start seeing improvements in the page's rankings).

If your page stayed the same and the optimization score changed, it means that either the list of competitors became different or that some of your competitors edited content on their pages.
